What documents do we need? 1. The complaint and any associated correspondence (emails, letters, notes of conversations) 2. The practice complaints policy 3. Relevant records (including records, radiographs FP17’s, treatment plans, referral documents and correspondence) 4. The dentist’s contract and confirmation of whether an associate or practice owner 5. The dentists comments in writing on the treatment, their recollection of any conversations and issues raised in the complaint
